Left Flipper:

You get a new ball by getting a combo of 5, for this to trigger a second time your combo must go back down to 3, so you can get many balls on the table by getting the combo to 5 and back down to 3 again after.
The images are distorted by drawing them out and taking a photo of them on the screen with a camera and then reimporting them back onto the computer.
If you download the source and compile it into a program you can use a gamepad to control it.
